Death Certificates
All deaths that occur within the Town of Caledonia are officially recorded with the Town Clerk.
Certified copies of death certificates are available to eligible individuals for a fee of $10 per copy. Payment may be made by cash, check, or money order, payable to: Caledonia Town Clerk.
Application forms can be obtained from the Town Clerk’s office or downloaded here.
Who May Request a Certified Copy:
Certified copies or transcripts of a death certificate may only be issued to:
The spouse, parent, or child of the deceased
The lawful representative of the spouse, parent, or child
An individual with a New York State Court Order, demonstrating a need for confidential medical information
A municipal, state, or federal agency requiring the information for official purposes
