Death Certificates

All deaths that occur within the Town of Caledonia are officially recorded with the Town Clerk.

Certified copies of death certificates are available to eligible individuals for a fee of $10 per copy. Payment may be made by cash, check, or money order, payable to: Caledonia Town Clerk.

Application forms can be obtained from the Town Clerk’s office or downloaded here.

Who May Request a Certified Copy:

Certified copies or transcripts of a death certificate may only be issued to:

  • The spouse, parent, or child of the deceased

  • The lawful representative of the spouse, parent, or child

  • An individual with a New York State Court Order, demonstrating a need for confidential medical information

  • A municipal, state, or federal agency requiring the information for official purposes

FAQ's About Death Certificates

If the person passed away at home or in Caledonia, we will have the death record. If the person passes away outside of Caledonia, we will not have the record.

A Certified Death Certificate can be obtained for any death recorded in the Town or Village of Caledonia. The cost is $10.00 per copy.

A certified copy or a certified transcript of a death certificate may be issued only to: the spouse, parent or child of the deceased, to a person with a New York State Court Order issued on a showing of necessity for the confidential medical information or by a municipal, state or federal agency specifically requesting the confidential medical information for official purposes.

No. The Town of Caledonia only provides certified copies of certificates. We will not provide a copy without a certification and raised seal. Photocopies are also not given out.

Yes. The Town of Caledonia needs you to fill out this application. Please note, we also need some form of identification with your application, which is spelled out on the application.
